Fillable forms with drop down menu

Our company would like to be able to run per reviews through this system as well. I would like to be able to build this form directly into Litmos rather than drag and drop a copy of the form. I would like to be able to add a drop down menu to the form. So think of it like a fillable PDF but, built using Litmos and you could then send the form out to select employees/teams. Litmos does not have the function to build out forms like this at this time.
